Who should use this form

You can use this form to help people whose disability benefit award has moved from Disability Living Allowance to Adult Disability Payment.

For example, if you work for a third sector or voluntary organisation that helps people to apply for benefits and complete forms. This help could include:

• filling in the form on someone’s behalf
• referring to the questions in the form if you’re helping someone to fill it in by phone or video call

This form will help to ensure that they receive the correct payments.

Returning the form

You should return the form in a pre-paid envelope, if you filled it in for the client. If you do not have an envelope, call us free on 0800 182 2222 and we’ll send you one.

Sending supporting information

Uploading documents online is the fastest way to send us supporting information.

You can also post copies of supporting documents to us. Supporting documents can go in the same envelope as the review form, if you’re filling in the form for the client.

If the client did not sign the form

We’ll send the client a copy of their completed form in the post, if they were not able to sign the form. This lets them read, sign and return their form.

Black and white version

A black and white version of the review form is available. This version cannot be edited on a computer. This form should be printed and then filled in using a black pen.
