Pension Age Disability Payment application form for organisations

Who should use this form

You can use this form to help people apply for Pension Age Disability Payment. For example, if you work for a third sector or voluntary organisation that helps people to apply for benefits. This help could include:

  • filling in the form on someone's behalf
  • referring to the questions in the the form if you're helping someone to fill it in by phone or video call

If you’re not completing this form as part of your job

Do not use this form if you’re:

It’s important we know the date you start your application. This form will not show that. We usually backdate your payment to that date, if your application is successful.

Before filling in the form

Before filling in the form, ask the applicant to:

We may be able to pay the applicant from the date they tell us they’re applying, if:

  • their application is successful
  • they've met the eligibility criteria including the qualifying period at this point

Returning the form

The form should be returned in the pre-paid envelope that is provided to the applicant. If the applicant does not have this envelope, they can call us free on 0800 182 2222 and we’ll send them one.

We are currently unable to send envelopes to third sector or voluntary organisations.

Sending supporting information

Uploading documents online is the fastest way to send us supporting information . You can also post copies of supporting documents to us. Supporting documents can go in the same envelope as the application form, if you’re filling in the form for the applicant.

Do not upload application forms online, application forms must be returned in the pre-paid envelope, otherwise this may delay the application.

If the applicant did not sign the form

We’ll send the applicant a copy of their completed form in the post, if they were not able to sign the form. This lets them read, sign and return their form. They’ll still get payments from the date they told us they were applying.
