A registered medical professional/registered nurse must complete and return the BASRiS form and the BASRiS Fee form to be able to claim a fee.
You can only claim a fee for a BASRiS form that has been submitted to Social Security Scotland along with the BASRiS Fee form.

On the home page, you will be asked which form you wish to complete. Select BASRiS fee web form.
You can download an editable BASRiS Fee form.
You should email completed editable BASRiS Fee forms to the addresses below:
For children (ages 0-16) return to: CDPBASRiS@socialsecurity.gov.scot
For adults (ages 16 to pension age) return to: ADPBASRiS@socialsecurity.gov.scot
You can do this at the same time you email an editable BASRiS form, or after.
Don’t worry if you submit forms to the wrong email address. We will re-route these internally.
We can only accept pdf BASRiS forms and BASRiS Fee forms that have been completed digitally and submitted via email. Do not complete these by hand, or post these.
You must send us a completed pdf BASRiS Fee form from an approved email address/domain name that is on our whitelist.
Accepted domain addresses can be found on our Requesting a BASRiS form guidance within the editable pdf dropdown section.
Stakeholders who are not on our whitelist will receive an automatic reply telling them their email has been rejected.
Stakeholders on our whitelist will receive an email acknowledgement when we receive the email.
If you do not receive an acknowledgement, please check your junk email to make sure your email has not been rejected.
If you wish to complete a BASRiS form or BASRiS Fee form by hand, or submit via post. Please use the paper forms.
Your BASRiS fee form claim will show on your remittance advice from the Scottish Government.
This document contains the date of the claim, the reference number for the claim and the BASRiS fee amount paid.
The reference number comprises of the GMC ID (6 digits), Patient CHI Number (10 digits) and the claim date (year-month-day).
