Requesting a BASRiS form

The 'allow list' in this guidance was formerly known as 'whitelist'.

The BASRiS form gives confirmation of an individual’s terminal illness for the purpose of accessing disability assistance benefits in Scotland.

If you have access to the Scottish Wide Area Network (SWAN) on an NHS network, you can complete a BASRiS web form. If you do not have access to SWAN you should use an editable PDF BASRiS form or a paper BASRiS form.

A new version of the BASRiS form was released on 15 December 2022. Please ensure you use the most up to date form.

The new form states that the date to be provided is the date the clinical judgement of terminal Illness was made. You should complete the BASRiS as soon as you make the clinical judgement, but if you complete it on a later date, you must record the exact date on which you made the clinical judgement.

Paper BASRiS forms completed by hand must be posted. Send a completed paper BASRiS form to:

Old BASRiS forms that do not explicitly ask for the Date of Clinical Judgement of Terminal Illness should not be used.

Who can submit a BASRiS web form

Registered medical practitioners or registered nurses with access to SWAN on the NHS network.

How to submit a BASRiS web form

  1. Using an NHS device, click the link https://basris.socialsecurity.gov.scot/
  2. On the home page, you will be asked which form you wish to complete.

    Select the type of form you wish to complete:
     
    1. BASRiS web form
    2. BASRiS Payment form after submitting a BASRiS web form

  3. Fill out the applicant’s details to the best of your knowledge.
  4. Tell us what you know about the applicant’s conditions and whether or not they are aware of their diagnosis and prognosis.
  5. Tell us your clinical judgement about the applicant’s condition. Record the date of clinical judgement of terminal illness.
  6. Complete the ‘Declaration of legal consent’.
  7. Fill out your details as a registered medical practitioner/registered nurse.
  8. Check over all your answers on the final page of the form then press ‘Continue’.
  9. By pressing ‘Continue’ at this stage, your form will be automatically submitted.
  10. Download and save a copy of the submitted form.
    1. You now have the option to fill out a BASRiS Payment form.
    2. you can only claim a fee for a BASRiS form that has been submitted to Social Security Scotland
    3. the BASRiS form and the BASRiS Payment form do not need to be submitted at the same time
    4. a fee is not paid for a verbal BASRiS submitted over the phone, it must be followed up with a BASRiS webform, PDF or paper form
    5. you must submit the BASRiS form confirming the verbal BASRiS within 28 days
    6. you’ll receive a letter requesting a signed BASRiS form if you’ve not submitted it within 28 days of submitting the verbal BASRiS

Title your email: BASRiS Error

Where possible also include the following:

  • name of your organisation
  • details of the error you have encountered
  • web address that the error was present on
  • browser, version number and operating system of your device
  • screenshots may be helpful, but any personal information should be redacted before submission.

Once your email has been received, we will contact you within 14 days to respond to your query.

If you are unable to use the webform, you can submit an editable PDF BASRiS form or a paper BASRiS while we investigate your issue.

How to send editable BASRiS forms and BASRiS Payment forms

You must email completed digital editable PDF BASRiS forms and BASRiS Payment forms to the addresses:

Don’t worry if you send forms to the wrong email address. We will re-route these internally.

You cannot print and post a copy of the editable BASRiS form. Only paper BASRiS forms can be submitted by post.

We can only accept pdf BASRiS forms and BASRiS Payment forms that have been completed digitally and sent by email. Do not complete these by hand, or post these.

If you wish to complete a BASRiS form or BASRiS Payment form by hand, and send it by post, please use the paper forms.

Stakeholders who are not on our allow list will receive an automatic reply telling them their email has been rejected. Check your junk email to make sure your email has not been rejected.

How to get on the BASRiS allow list

Our BASRiS allow list is a list of all professionals and professional organisations who can send a BASRiS form and claim a fee for doing so.

If you are not on the allow list then first confirm your organisation is on The Scottish Charity Register (OSCR). Only an organisation on the OSCR can be accepted onto our allow list.

If your organisation is on the OSCR then you can apply to join our allow list by emailing:

In your email, we need you to:

  • state the name of your organisation
  • tell us the domain name to be added to the allow list (i.e. nhs.com)
  • give us contact details for the person applying to join the allow list on your organisation’s behalf

You’ll receive an acknowledgement that your email has been received. We’ll then contact you within 14 days to either confirm you’ve been added to the whitelist or advise you why your application has been rejected.

Request paper forms

If you're a medical practitioner you can ask for paper BASRiS forms by filling out the form on GOV.UK. If you're an applicant and think you need a BASRiS, you should ask a medical practitioner for one.
